How to Become a New Patient at White's Pediatrics
Step 1:
Schedule Your Child's First Appointment
Call White’s Pediatrics at (706) 876-2130 to get started. Let our team know you’d like to establish care as a new patient, and we’ll help you choose the right visit type and walk you through the next steps.
Please be ready to share:
- Your child’s name and date of birth
- Parent or guardian contact information
- Insurance information
- Reason for the visit
- Any immediate medical concerns
Step 2:
Complete Your New Patient Forms
Before your first visit, please complete the required new patient paperwork. These forms help us gather the information we need to begin caring for your child and make your first appointment as smooth as possible.
Paperwork may include:
- Patient history forms
- HIPAA and privacy acknowledgment forms
- Financial policy forms
- Appointment agreement forms
- Any immediate medical concerns
Transferring from another practice? A parent or legal guardian will also be asked to sign a Release of Information form so we can request your child’s medical records from their previous pediatrician.
Step 3:
Gather What You'll Need for Your First Visit
To make check-in quick and easy, please bring the following to your appointment:
- Completed new patient forms
- Insurance card
- Parent or guardian photo ID (driver’s license or government-issued ID)
- Relevant medical records, if available
- Release of Information form, if transferring from another pediatrician
- Any additional documents requested by our team.

Step 4:
Arrive Early for Check-In
Please arrive 15 minutes before your scheduled appointment time. This gives our team time to:
- Review your completed paperwork
- Verify insurance coverage
- Confirm contact information
- Get your child ready to be seen.
Arriving early helps keep your visit and the appointments that follow — running smoothly for every family.
Step 5:
Meet the Team and Establish Care
Once check-in is complete, your child will meet with a White’s Pediatrics provider and your family can begin building a trusted, long-term relationship with our practice.
During this first visit, we’ll review important information with you, answer any questions you have, and make sure your child’s care is off to a great start.
